What fields does a proof of delivery form need?
Six of them at minimum: who shipped it, who received it, what was received and how much of it, the date and time it changed hands, any exception noted at the door, and a signature from the person accepting the goods. Drop any one and the document answers a weaker question than it looks like it answers.
Is there a proof of delivery template for Word?

Is a proof of delivery form the same as a bill of lading?
No. A bill of lading is issued at pickup and describes what a carrier agreed to haul. A proof of delivery form is completed at the other end and records what actually arrived and in what condition. Freight shipments usually use both; a local delivery run often uses only the second.
Does a signed proof of delivery form replace carrier tracking?
No, and the two solve different problems. A tracking number is timestamped by a party neither side controls; a paper form is only as good as whoever filled it out. Use the form for deliveries with no carrier account behind them, and use tracking whenever a label already exists.
